Medical Director
Dr. Bergman is a Certified Medical Director through the American Board of Post-Acute and Long-Term Care Medicine (ABPLM). Dr. Bergman is a rehabilitation specialist/long-term care primary care physician practicing in nursing homes. He cares for older patients who have been recently discharged from the hospital and are recovering from complex medical and surgical needs, they often lack the ability to make decisions or understand their condition, and are living with long-standing mental and physical ailments.

Dr. Farooq is a Certified Medical Director through the American Board of Post-Acute and Long-Term care medicine (ABPLM). He completed his internal medicine residency from The Mount Sinai/ Ball Memorial Hospital. In addition, he completed a pulmonary medicine/sleep medicine, fellowship from the Western Pennsylvania hospital. Dr. Farooq is board-certified by the American Boards of internal medicine, pulmonary medicine, and sleep medicine. He provides dedicated care in skilled nursing and in long term care.
