Dr. Carl Christian Bergman, MD

Dr. Bergman is a Certified Medical Director through the American Board of Post-Acute and Long-Term Care Medicine (ABPLM). Dr. Bergman is a rehabilitation specialist/long-term care primary care physician practicing in nursing homes. He cares for older patients who have been recently discharged from the hospital and are recovering from complex medical and surgical needs, they often lack the ability to make decisions or understand their condition, and are living with long-standing mental and physical ailments.

Dr. Mohammad Farooq, MD, FCCP, CMD

Co-medical Director

Dr. Farooq is a Certified Medical Director through the American Board of Post-Acute and Long-Term care medicine (ABPLM). He completed his internal medicine residency from The Mount Sinai/ Ball Memorial Hospital. In addition, he completed a pulmonary medicine/sleep medicine, fellowship from the Western Pennsylvania hospital. Dr. Farooq is board-certified by the American Boards of internal medicine, pulmonary medicine, and sleep medicine. He provides dedicated care in skilled nursing and in long term care.

Education: Master of Science in Nursing and Bachelor of Science in Nursing from Virginia Commonwealth University; Diploma in Nursing from Bon Secours Memorial School of Nursing; Awards: Excellence in Clinical Practice Nominee from VCU Department of Internal Medicine January 2020; March of Dimes Virginia Critical Care Nurse of the Year Finalist. Memberships: Gerontological Advanced Practice Nurses Association Member.

Psych Nurse Practitioner

Board certified Adult-Gerontology Nurse Practitioner with 10 years of experience in nursing and 9 years of military leadership experience, seeking a position that focuses on patient assessment, treatment planning, education, and staff management.
