Medical Director
Dr. Bergman is a Certified Medical Director through the American Board of Post-Acute and Long-Term Care Medicine (ABPLM). Dr. Bergman is a rehabilitation specialist/long-term care primary care physician practicing in nursing homes. He cares for older patients who have been recently discharged from the hospital and are recovering from complex medical and surgical needs, they often lack the ability to make decisions or understand their condition, and are living with long-standing mental and physical ailments.
NP for VCU
Janaya Ogunware is a board-certified nurse practitioner who has been serving in the healthcare field for more than 20 years. She has worked as a nurse in the Richmond area for VCU Health System since 2005 and received her master’s degree in nursing from Virginia Commonwealth University in 2010. Janaya has experience as a nurse practitioner managing complex patients in a variety of specialties to include internal medicine, cardiothoracic surgery, trauma, and post-acute care. Janaya joined the nursing facility attending service with the Division of Geriatric Medicine and continuum care services at VCU Health System in 2023.
Physician
Dr. Mohammed Farooq is a Certified Medical Director through the American Board of Post-Acute and Long-Term care medicine (ABPLM). He completed his internal medicine residency from The Mount Sinai/Ball Memorial Hospital. In addition, he completed a pulmonary medicine/sleep medicine, fellowship from the Western Pennsylvania hospital. Dr. Farooq is board-certified by the American Boards of internal medicine, pulmonary medicine, and sleep medicine. He provides dedicated care in skilled nursing and in long term care.
Psych Nurse Practitioner
Board certified Adult-Gerontology Nurse Practitioner with 10 years of experience in nursing and 9 years of military leadership experience, seeking a position that focuses on patient assessment, treatment planning, education, and staff management. Education: Master of Science in Nursing from South University, Savannah, GA; and Bachelor of Science in Nursing from Fayetteville State University – Fayetteville, NC.
Physician
Dr. Erica Carter earned a Bachelor of Science in Biology with a minor in Chemistry at Virginia Commonwealth University. Dr. Carter completed her internship and medical residency at Carolinas Medical Center Northeast, in Family Medicine. She served as Chief Resident during her last year. As a board-certified Family Medicine Physician with nine years of experience as a hospitalist physician, she is well versed in caring for complex patients. She also has over three years of experience working at a skilled nursing facility, caring for rehabilitation patients, as well as long term care patients.
